Welcome to the Medical Records and Health Management Department. We provide information for all hospital, nursing home, and physician services.
There are several ways to access your medical records, so you can choose what works best for you.
Your patient portal has the most up-to-date information on your Laboratory, Imaging, and Emergency Room visits.
You can see and print your results from your location 24 hours after the results are completed!
You can complete our online form to easily and securely request your records.
Come to our office at 228 N. Cherry Street, Fruita, CO. 81521 to file an in-person Release of Records Authorization Form. We are open from 8 a.m. – 5 p.m., Monday – Friday. We are closed on holidays and weekends.
