Dr. Evans is joining Family Health West Primary Care, and we’ve made it easy to keep her as your provider!

As someone who truly values her patient relationships and enjoys providing healthcare to the whole family, Dr. Evans is excited to be joining the FHW Primary Care team. It’s important to Dr. Evans to be part of a clinic that is integrated and supportive of the Grand Valley community. 

If you would like to keep Dr. Evans as your primary care provider, simply fill out our online form to transfer your records. Transferring your records ensures there is no gap in your care. Our forms are easy to use and HIPAA Compliant.

Keeping Dr. Evans as your primary care provider is easy. Simply fill out our HIPAA-compliant form, and we’ll take care of the rest. Your records will be transferred to FHW Primary Care so your care can pick up right where it left off.

Dr. Evans’ will start at FHW Primary Care in June 2023. If you choose to keep her as your primary care provider and need an appointment before then, one of our other primary care providers would be happy to see you in the meantime.

Dr. Evans is accepting new patients! If you are not already a patient of Dr. Evans but would like her to be your primary care provider, you can request her as your new doctor. Just fill out our new patient request form and be sure to select her as your preferred provider.

Dr. Evans describes herself as someone drawn to medicine because of a desire to combine her passion for science with a hands-on approach to improving every patients’ life. She chose family medicine because of the variety of medicine and patient ages she sees as a family physician, along with the desire to develop longitudinal relationships with patients. Professionally, Dr. Evans is focused on staying up-to-date with evidence-based medicine and thus being able to provide her patients with the best recommendations possible. She also enjoys and prioritizes assisting patients in finding the healthiest version of themselves with a focus on preventive medicine. She had an experience in medical school that acknowledges has helped her realize how important it is to just listen to her patients and meet them where they’re at.

She says it helps her develop strong and meaningful relationships with her patients so that I can make them feel heard and provide them with the best care possible. That experience has carried with her throughout her time as a physician in treating each and every patient.
