Family Health West knows that paying for medical care can be difficult for many families. As part of Family Health West Hospital’s commitment to our patients, we provide counseling and assistance to help individuals apply for available programs that may help cover unpaid hospital bills.

  • Patients who are uninsured or underinsured may qualify for PEAK (Colorado Medicaid) or HDC. We can provide the necessary information for you to apply for assistance.
  • If you are not eligible for PEAK or HDC, our Financial Counselor can discuss other options available to you, such as a discount at the time of service for self-pay patients or monthly payment plans.

Application forms and information are listed below. If you receive (or plan to receive) medical care at Family Health West hospital or a FHW owned clinics and are interested in payment options, call 970-200-1714 or email [email protected]. You can apply for financial assistance in person or by mail.

Hospital discounts for uninsured patients

If you need help paying a hospital bill, you can see if you qualify for
discounted care. You may qualify for discounted care if you are at or below
250% of the Federal Poverty Guidelines (FPG). If you qualify, hospitals and
providers must limit your bills and offer a payment plan based on your income.

Colorado Indigent Care (CICP) ends July 1, 2025

The Colorado Indigent Care Program (CICP) will be ending on July 1, 2025. A
list of resources is included below to help you. Please contact your place of
care to ask about other discounted care options they may have. Our team is
available to answer any questions you have about Family Health West
Hospital Discounted Care or other financial assistance options available at our
facility. You can reach us at 970-200-1714.

  • Your co-pay card may not be valid after CICP ends. Talk to your place of care
    for more information.
  • For CICP clinic patients – check with your clinic about financial assistance
    options.
  • Clinics may continue to give co-pay cards for their own programs, but some
    clinics will not take these cards after July 1, 2025.
  • For CICP Hospital patients – if you qualified for CICP through a hospital you automatically qualify for Hospital Discounted Care. Hospitals must provide a determination letter to their patients after they apply for Hospital Discounted Care. These determination letters can be used at other hospitals.
  • The hospital’s financial assistance team will help you fill out an application for
    Hospital Discounted Care.
