Around-The-Clock Care:  You’ll have 24/7 physician supervision, direct access to multidisciplinary specialists, emergency care, and on-site lab, imaging, and respiratory therapy services.

Better Outcomes: Our Physical Therapist Team spends 5 hours and 49 minutes with each patient daily, compared to the national average of 5 minutes. Our targeted and individualized rehabilitation service is offered seven days a week, helping you to return home 10 days sooner than other facilities.

Personalized Care: With the largest therapy team in Western Colorado, we are able to provide one-on-one therapy and an exceptionally high nurse-to-patient ratio of 1:5. Our nurses spend more than 9 hours with each resident each day, compared to the national average of 41 minutes.

Comfortable Atmosphere: During your stay, you’ll enjoy delicious meal options prepared by our award-winning dietary team with a full menu. You can also partake in social activities such as arts and crafts, cooking classes, and games.

Driving rehabilitation was designed to help achieve independence after a traumatic injury and safety for aging drivers. 

Our vehicle can be adapted to meet each driver's unique needs, and we can also help modify your vehicle to meet your driving needs.

In addition to our physical, occupational, and speech team, respiratory care therapists are available around the clock to help you breathe easier. You’ll find all the respiratory services you need in one on-site team, from sleep studies to pulmonary exercises.

Medicare generally covers transitional care. We accept most Medicare Advantage plans and work to get pre-approval for most commercial insurance providers.

Family Health West Hospital, located in Fruita, Colorado, serves patients from all of Mesa County, including Grand Junction, CO, and various surrounding areas.

Knowing that having privacy is peace is an essential part of recovery, we’ve designed comfortable, private contemporary rooms, each with a private bathroom and shower.
