Transitional care is a type of specialized rehabilitation ideal for patients who need supervised rehabilitation before returning home after a hospital stay. For patients recovering from major surgery, or who are dealing with chronic health conditions, this supervised rehabilitation can help patients return home and live on their own again.

Our approach to transitional care is special. We combine the benefits of a skilled nursing home, an intensive rehabilitation program, and an acute care hospital into one unique program.
Around-the-clock-care: You’ll have 24/7 physician supervision, direct access to multidisciplinary specialists and emergency care and 0n-site lab, imaging, and respiratory therapy services.
Better outcomes: Our Physical Therapist Team spends 5 hours and 49 minutes with each patient per day, compared to the national average of 5 minutes. Our targeted and individualized rehabilitation service is offered seven days a week, helping you to return home 10 days sooner than other facilities.
Personalized Care: With the largest therapy team in western Colorado, we’re able to provide one-on-one therapy and an exceptionally high nurse-to-patient ratio of 1:5. Our nurses spend more than 9 hours with each resident each day, compared to the national average of 41 minutes.
Comfortable Atmosphere: During your stay, you’ll enjoy delicious meal options prepared by our award-winning dietary team with a full menu to choose from. You can also partake in social activities such as arts and crafts, cooking classes, and games.
Skilled Nurses: Our Skilled Nursing team specializes in transitional rehabilitation. Their entire focus is on helping you recover safely and effectively to help you bridge the gap between the hospital and home.
On-site Emergency Department: Patients have access to an on-site emergency department to address and any problems that might arise. In the event of an emergency, it’s a trip down the hall instead of in an ambulance.

Yes, your physician will refer you to a skilled nursing program after being discharged from the hospital. But, you can choose which facility you prefer to receive continued care. Make sure you tell you physician you want to come to Colorado Canyons Hospital.
Medicare generally covers transitional care. We accept most Medicare Advantage Plans and work to get preapproval for most commercial insurance providers.
